This position manages the overall responsibilities of the person-centered planning (PCP) process for Divine Care, Inc.
This position will maintain the annual schedule of all Person-Centered Plans while ensuring all budget-related staffing provisions and supporting documentation are in place in accordance with our practices in the electronic documentation system.
Interactions are timely, responsive, and courteous to all stakeholders.
Actively initiates compliance efforts in scheduling, documentation submission and clarification requests.
Demonstrates respect and sensitivity for each person's individuality and preferences along with the cultural/ethnic diversity of the people served.
Promotes and protects each person's rights, privacy, and confidentiality.
Works collaboratively with others to accomplish goals/tasks and deadlines Reports To:
Executive Director Duties and Responsibilities Interact with staff and external entities in a timely, responsive, and courteous manner.
Addresses complaints identified during the PCP process and actively engage in problem-solving.
Perform all routine administrative duties throughout the PCP process.
Review all incoming correspondence (external emails, supporting documentation required for scanning submission in electronic documentation system).
Actively promote and/or protect each person's rights, privacy, and confidentiality.
Participate in all required and scheduled meetings and record of meetings through minutes.
Conducts individual specific training in conjunction with respective Program Manager/Coordinator.
Keeps abreast of all PCP-related policy changes within DDA.
Ensuring funding streams and limits are updated at least annually or as needed in the electronic documentation system.
Establish and maintain effective working relationships with family members of those we serve as well as Coordinator of Community Supports (CCS) and other providers.
Monitor progress and data collection for outcome, goals, and services in the PCP monthly, quarterly, and annually.
Provide regular updates and reports to Chief Operating Officer and Executive Director regarding PCP status and budget-related clarifications.
Conducts Quality Assurance audits on all PCP components not limited to outcomes, goals, and other services.
Works alongside Delegating Nurses to ensure timely HRST review.
Complete PCP pre-planning collaboratively with CCS and other team members.
Act as the point person for conducting Support Intensity Scale (SIS) as prescribed times.
Complete quality monitoring checklist of PCP prior to authorization of services.
Obtain signatures for PCP for all parties required.
Maintaining updated information in electronic documentation system (i.
e.
, Face sheet, Providers and Emergency Contacts, etc.
).
Review documents needed for intake during the admissions process.
Maintain budgetary worksheets for scenarios in funding (i.
e.
, 1:
1, 2:
1 staffing augmentation, Community Integration, and other dedicated hour needs).
Completion of the Detail Services Authorization Tool (DSAT), Service Implementation Plan (SIP), Cost Detail, Personal Supports schedule, Community Integration schedule, and 2:
1 Staffing schedules.
Serve as the point person for behavioral consultation and provide supporting documentation.
Present Behavior Support Plans (BSP) at Divine Care, Inc's.
Standing Committee.
Build a profile of Person-Centered Thinking Tools based on the person's needs.
Maintain Final Planning provisions for each person served.
Send correspondence to families of Personal Support and/or Respite to obtain missing information.
Update Risks and Restrictions in PCP to ensure that aligns with nursing care plan, behavior support plan, and any other source document for compliance.
Ensure diagnoses and other relevant information (ex:
allergies) are consistent with nursing care plan, behavior plan, 45-day reviews, MAR's, and PMOFs.
Ensure solicitation with CCS on completed PCP with signatures via email.
Manage incident reporting process and documentation requests related to investigation practices.
Act as the liaison (Point of Contact) between Divine Care and external agencies regarding referrals and admissions.
Solicit interest in obtaining referrals for admissions and attend Transitioning Youth platforms for recruitment.
Electronic Documentation Platform Orientation Other related duties as assigned.
