Summary A Care Coordinator's responsibilities include consulting with patients and determining their needs, developing an individualized plan of action that includes coordination from all parties involved in order to provide the best possible patient care.
Duties and Responsibilities Assists all patients through the healthcare system by acting as a patient advocate and navigator Educate patients about their condition, medication, and give them specific instructions.
Develop a care plan to address their personal health care needs.
Consult and collaborate with other health care providers and specialists to set up patient appointments and treatment plans.
Educates patients and their families on the care plan; provides details related to treatments, procedures, medications, and continuing care requirements.
Coordinates continuity of patient care with external healthcare organizations and facilities, including the process hospital admission and discharge and referrals from the primary care provider to a specialty care provider.
Check-in on the patient regularly and evaluate and document their progress.
Assist the care team with developing and assessing health interventions.
Attend ongoing training and courses to keep abreast of new developments in health care.
Maintains patient care database by entering new information as it becomes available.
Conducts comprehensive, preventive screenings for patients and/or assists all support staff in daily patient interactions as needed.
Facilitates patient medication management based upon standing orders and protocols.
Treat patients with empathy and respect and conduct oneself in a professional manner.
